  You must be at most 39 years old.
• As a man, you are at least 25 years old.
  You must be at most 49 years old.

The health insurance company pays 50 percent of the treatment.

This means:

You get half of the money back.

Important:

• A doctor must write the treatment plan.
• They must show the plan to the health insurance company before the first treatment.
• The health insurance fund must approve the treatment plan.

You submit the application to your health insurance fund.

Many health insurance companies allow you to do this online.

Important:

• The woman's health insurance fund pays for the treatments on the woman's body.
• The man's health insurance pays for the treatments on the man's body.

You will need documents for the application.

For example:

• The treatment plan from the doctor.
• Or the treatment plan from the Children's Wish Center.
