Your identity card, passport or other recognized passport replacement document of both parents

To establish the credibility of the miscarriage:

a certificate of miscarriage issued by a doctor or midwife/obstetrician or
the maternity record, if it shows the miscarriage
if applicable, a certificate of burial of the miscarriage
if possible: the birth certificates of the parents or your marriage certificate

For the preparation of the certificate:
